  • The aim of this retrospective analysis was to summarize the course of fecal zonulin levels in a shared dataset of 128 patients undergoing a NANA10-based intervention.

  • and a Kaplan–Meier-like presentation of the normalization of pathologically elevated zonulin levels.

  • For the primary analysis, paired values at time point 1 (Z1) and Z2 were evaluated in n=128 patients.
  • Additional available longitudinal values up to Z7 were also considered.

3) Ergebnisse

  • The median fecal zonulin level decreased from 179.5 ng/ml at Z1 to 82.7 ng/ml at Z2.
  • The Wilcoxon signed-rank test for paired data showed a highly significant decrease from Z1 to Z2 (n=128; p=2.48 ×10^-17).
  • The median of the absolute differences was 85.0 ng/ml, and the median relative change corresponded to a decrease of 56.5%.
  • In the sign test, 109 of 128 individuals showed a decrease (p=7.78 × 10^-17).
  • The proportion of patients above the reference limit of 131 ng/ml decreased from 59.38% at Z1 to 21.09% at Z2.
  • The exact McNemar test confirmed this reduction with p=5.91 × 10^-12.
  • Among 48 complete cases with Z1–Z3 values, the Friedman test showed a significant multi-time-point difference (chi-square=45.33; p=1.43 × 10^-10).
  • A log-linear time model using all available measurements yielded a multiplicative monthly factor of 0.95485,
  • corresponding to an estimated decrease of 4.52% per month (p=4.11 × 10^-6).
  • In a Kaplan–Meier-like assessment, the proportion of pathologically elevated values above 131 ng/ml

  • In einer retrospektiven, praxisbasierten Follow-up-Analyse wurden 128 Patientinnen und Patienten mit chronischen Gesundheitsproblemen und einer zonulinbezogenen klinischen Fragestellung ausgewertet.
  • Unter einer NANA10-basierten Intervention in Kombination mit einer Vitamin-D-Optimierung sank der Median des fäkalen Zonulinspiegels von 179,5 ng/ml auf 82,7 ng/ml.
  • Der Anteil oberhalb der Referenzgrenze von 131 ng/ml verringerte sich von 59,38 % auf 21,09 %.
  • Gepaarte nichtparametrische Tests, der McNemar-Test, der Friedman-Test sowie ein log-lineares Zeitmodell zeigten konsistent hochsignifikante Abnahmen.
  • Longitudinalanalysen sprechen für eine weitgehend nachhaltige Reduktion;
  • eine Kaplan-Meier-ähnliche Darstellung weist auf eine substanzielle Normalisierung pathologischer Werte über einen Zeitraum von etwa 18 bis 20 Monaten hin.
  • Aufgrund des retrospektiven, unkontrollierten Designs sind die Ergebnisse hypothesengenerierend und sollten in prospektiven kontrollierten Studien überprüft werden.
